| Under "Physical Cards" click on the appropriate student-athlete physical card. Note: Only the Medical Eligibility Form (p. 4) should be submitted to the school. *Athletes must have a current sports physical on file at NLS in order to participate in a school sport. Physicals are valid for two years from the date of exam. |
